How to fill out the Odometer Statement form

This Odometer Disclosure Statement form contains three sections and below I’ll explain how to fill ot this generic form:

Section 1

The first section includes the full name of the seller. Particularly the seller’s name must pe PRINTED. This means to use only Capital Letters to write SELLER’S NAME. Also complete the seller name(s) exactly as it appears on the Certificate of title. Then fill out the vehicle details starting with the current odometer reading. However if the odometer reading is in excess of the mechanical limits of the odometer or is not the actual mileage please check the corresponding box:

Section 2

Please take into consideration that only one seller (owner) must sign if the title states OR between owner names printed on the title. Both sellers (owners) must sign if the title states AND or AND/OR between owner names, or if nothing is listed between owner names.

Next fill out in the second section the Seller (Transferor) Information. This includes the seller’s signature and name. Please write SELLER’S NAME using only Capital Letters. Then fill out seller’s address:

Section 3:

Lastly in the third section fill out the Buyer (Transferee) Information. This includes the buyer’s signature and name. Please write BUYER’S NAME using only Capital Letters. Next fill out the buyer’s address, and date of certification:
